Steps to Follow:

  1. If you have not downloaded the Licensed Professional Evaluation PDF Form to be completed by a Licensed Professional under Step 1, you will need to do that now.
  2. Fill the form below accurately.
  3. Attach along the completed Licensed Professional Evaluation Form with this submission.

We ask all candidates requesting an accommodation to take the time to carefully complete this form before submitting your request. A completed Accommodation Request includes the Candidate Accommodation Request Form, the Professional Evaluation Form, and any additional information or documentation requested by Prometric and/or your Test Sponsor in order to evaluate an accommodation request. A complete form will allow Prometric to assist the candidate in arranging the best accommodation possible for the situation. We are unable to process incomplete accommodation requests.
