Up-to-Date Contact Information 

After scheduling your exam appointment, Prometric will send you a confirmation email with your scheduled exam date, time, and location. To ensure that you receive your exam appointment information via email from Prometric, please keep your contact information current.  

• To update your email address, please sign in to http://www.asq.org and click on My Account. 
• If you use your work email address, please keep in mind some employers may block receipt of employees' personal email.  
• Please also keep in mind some spam protection software may move the e-mail to a spam folder, so please take steps to ensure you will receive any email regarding your certification exam. 

Required Identification

To gain entry into the Prometric testing center or to take a live, remote proctored exam, you must present a valid and current (i.e., nonexpired) form of government-issued identification, such as a driver’s license or passport. The first and last name on your ID must exactly match the name on your seating notification (i.e., email confirmation) from Prometric. If you need to update your name to match your photo ID, please contact ASQE at cert@asq.org. Provide the following information exactly as it appears on the government-issued identification you will bring to your exam appointment with Prometric:

• First name (Given name) 
• Middle name 
• Last name (Family name/Surname)  
• All name changes must be submitted at least 6 business days before your Prometric exam appointment.
