FLDOH Testing Information - Learn more about the tests offered by Prometric by visiting the FLDOH Web site or by clicking on the links below:

Parental/Guardian Release Form - This document is mandatory for minor candidates. It requires a signature from a parent or guardian authorizing the minor to take the exam, acknowledging the testing rules, and, if necessary, appointing a designated caregiver to accompany the child.

Bulk Scheduling - Group Registration requests can be submitted by email to PTCteam@prometric.com ONLY. Our hours of operation are Monday-Friday from 8am-5pm EST.

The following information must be included in the initial group registration request via email sent:

  • Group Requestor/Client Name and Organization
  • Group Requestor/Client Email Address and Phone number
  • Exam Name/Code
  • Preferred/Alternate Test Dates- 1st, 2nd, 3rd
  • Preferred/Alternate Test Locations- 1st, 2nd, 3rd

Important Notice:

Ensure that when you are scheduling for your exam, you will use the identifier that is noted within your ATT Letter.

Beginning on April 9th, your ATT Letter will be changed, as you will no longer be using your Social Security number as your Eligibility ID. Your ATT Letter will provide you with a unique 9-digit ID that you will use when scheduling for your exam.

If you have received your ATT Letter PRIOR to April 9th, your ATT Letter will have the following verbiage “Eligibility ID number is your Social Security Number”, you will use your Social Security number when scheduling for your exam.
