Important Announcements
STOP: Pursuant to Section 456.0635, Florida Statutes, effective July 1, 2009, health care boards and/or the department shall refuse to issue a license, certificate or registration and shall refuse to admit a candidate for examination if the candidate has a conviction based on this new statute. Prometric is now required to collect additional background questions from candidates prior to scheduling their exam. The additional background questions are posted below under Certified Nursing Assistant Application Addendum . Candidates must complete both the Florida Certified Nursing Assistant Application and the Florida Certified Nursing Assistant Application Addendum prior to taking an exam.
