Prometric now offers duplicate, color photo insurance licenses as an optional service to licensed individuals in the state of Louisiana. The wallet-sized, plastic licenses are available for a fee of $30 per license type.

To request this duplicate license, select one of the following methods:

1. Complete the Request for Duplicate Photo Louisiana Insurance License and submit with payment to Prometric . After allowing mailing and processing time, call Prometric to make an appointment to have your photograph taken at an Prometric testing site*.
2. Call Prometric at 800-871-6457 to request the duplicate license and provide credit card information. You will make an appointment to have your photograph taken at an Prometric testing site*.
3. After your photograph is taken and the Louisiana Department of Insurance confirms license type and lines of authority to Prometric, your photo license will be produced and mailed to you.*

If you have taken an insurance exam within the last six months, you will not need to have a new photograph taken.
